Hulk Form Builder has made it simple for me to get information from my customers, with notification and their entries sent right to my email. The form was easy to for me to give a professional appearance and provided many options for customization (one of my fields is to select one of 50 states from a drop-down menu). Works great, its a simple and easy way to get form uploads in Shopify without having to do your own complex setup. I had an technical issue, but there support responded in 2 minutes and fixed it right away.
