Zigpoll is a great way to collect customer data on your website. You can customize quite a bit with their different survey options. Their support team is extremely helpful and responds right away. Highly recommend

Great app. We use the post-purchase survey to find out how our customers found out about us and it is really useful data to help us know where to concentrate our marketing efforts.
