Needed to change email address within system and Ankit understood the problem and was most helpful in sorting it all out

I have used EM Forms Builder for several years and it has performed as expected. After making some website changes I required some technical support and they worked with me quickly and efficiently to get everything working properly again.

The free version lets you create a form, but does not allow you to see the results when the form is completed. You have to upgrade, which of course makes the "free" plan useless.

We needed a form to enable customers to request a quotation for custom work to be done on their furniture. This app works very well, allowing customers to use various methods to describe their items and even attach photos to base our quotations on. There was an issue with how the form was displaying on our Shopify website at first, but I was very impressed with how quickly Form Builder/EVM's customer support focused on my issue and shortly after sent me an email stating that it was fixed. They quickly and perfectly cared for that hiccup and we're very happy with their results. This is a flexible solution that fits our needs. Well done!

Good app for a custom form. Customer service helped me with some issues I had using multiple forms and got it fixed promptly.

Great experience. I had an issue when first creating the form and the customer service was excellent and quick to respond and resolve the issue. They even followed up to ensure everything else went smoothly. I am using the free version and it’s perfect for what I need.

There are a lot of great features for this application and it was incredibly easy to produce a form myself. After I completed the form the .PDF file extension would not upload. I reached out to customer service, had an immediate collab response, and the issue was fixed within minutes. The only thing that I would like to see on this app is the ability to submit .DST, .AI, .EPS, .CDR, and other forms of vector or art extensions.

I used this app to create a custom form for customer product registration. Simple and straightforward process and our custom form works well. Their customer service responds quickly and took care of slight adjustments to our form quickly.
