Opus cart upsell is good. Its has many options to help suit it to your needs. The Opus team also helps you with modifications. The pricing is reasonable as well. The only drawback is the analytics sometimes aren't tracked well.

This is a great little app. I'm still learning some of the features but it mostly does what I need it to do. Upsell, discount codes, tiered rewards based on different criteria. Customizable cart look and feel. Plus so far the support has been great. I worked with Lulla to get answers to my questions and get updated to the latest version. Thank a bunch!
