Description

Add custom fields on the cart page. Request the information you need to fulfill orders.

With our app, you can easily add custom fields to your cart page, and optionally make them required. Customers won't be able to visit the checkout area unless they fill out the information you need. The captured information is saved with Shopify and available to you in the order page. It can also be exported, displayed in emails and order printouts etc. The app supports the newest theme format known as Online Store 2.0.

Field types: Text, Number, Checkbox, Radio, Dropdown, Combobox, and Text area.
Simple drag and drop install process, built into the Theme Editor.
Field data is saved in Shopify and easily accessible on the order page.
Efficient code served from Shopify's servers — minimal impact on store speed.
