Awful experience. No customer service / support whatsoever. I started using this for my Shopify site - worked well, but I needed a small tweak, to be able to have a form offer 40 choices rather than the standard 20. Emailed for support -- absolutely no response. It's been weeks. Shopify's customer support even emailed them, & still no response. Now I need to let go of 3-4 hours of work in setup, and try to find some other niche app that does the same thing. Really frustrating, do not recommend.
