$17.99 for 25 Survey Responses...? - Hold on... so you have a "Starter" package that costs $18 per month. And it only allows 25 form submissions per month!? That's unbelievably. Does anybody really pay for that? What does it cost you to allow them more survey responses? For $18 you should AT LEAST offer 250.
